Respite providers come into the home to provide caregivers a break or respite from the challenging task of caring for loved ones. Phase 2

SEKRS Employee Check Sheet (pdf): List of items that need to be completed and returned to SEKRS before beginning employment.

K-4 Kansas Employee's Withholding Allowance Certificate: A completed withholding allowance certificate will let your employer know how much Kansas income tax should be withheld from your pay on income you earn from Kansas sources. Please contact SEKRS if you need assistance with this form.

Form W-4: Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay.

Form I-9: Employment Eligibility Verification, Department of Homeland Security, U.S. Citizenship and Immigration Services: Acceptable documentation is listed, List A: Documents that establish both identity and employment authorization, List B: Documents that establish Identity plus List C: Documents that establish employment authorization.

Authorization for Payroll Direct Deposit:  Authorizes SEKRS and your financial institution to use payroll direct deposit. You will need to attach a voided check or savings account with this document.

Phase 3

SEKRS Employee Procedures and Policies (pdf)
Document contains the procedures and policies for all SEKRS, Inc. employees: Employment relationship policy; Code of ethics; Statement of confidentiality; Provision of service; Substance abuse policy; Abuses and neglect; HBV policies; Medication policies and procedures; HIPAA policy with quiz; Individual rights; Conflict of interest; Cell phone usage and texting while driving; Possession of fire arms; Overtime and holiday compensation. Requires a statement confirming that all policies have been read and understood. Also requires return of completed HIPAA quiz.
